Disbiosis cervicovaginal asociada a infección y persistencia del Virus del Papiloma Humano en mujeres revisión paraguas, 2019-2025

Se ha sugerido que la disbiosis cervicovaginal constituye un cofactor biológico modificable capaz de alterar la barrera mucosa y el microambiente inflamatorio, mientras que la infección persistente por el virus del papiloma humano (VPH) es una condición necesaria, pero no suficiente, para el desarrollo de cáncer de cuello uterino. El objetivo fue sintetizar y evaluar críticamente la evidencia de revisiones sistemáticas sobre la relación entre la microbiota cervicovaginal, la infección y persistencia del VPH y la progresión hacia neoplasia intraepitelial cervical y cáncer cervicouterino. Se realizó una revisión paraguas de revisiones sistemáticas con o sin metaanálisis, siguiendo PRISMA 2020 y PRISMA-S. Se buscaron estudios en MEDLINE/PubMed, Embase, Scopus, Web of Science, Cochrane, LILACS y Epistemonikos hasta el 3 de noviembre de 2025, en inglés, español y portugués. Dos revisores efectuaron de forma independiente la selección y la evaluación metodológica mediante AMSTAR 2; los hallazgos se sintetizaron de manera narrativa y temática. Se incluyeron ocho revisiones sistemáticas. La vaginosis bacteriana, la pérdida de dominancia de Lactobacillus y los perfiles no dominados por L. crispatus, especialmente CST IV y comunidades dominadas por L. iners, se asociaron con mayor probabilidad de infección por VPH, persistencia viral y lesiones cervicales de alto grado. Los perfiles dominados por L. crispatus mostraron, en cambio, una asociación con menor riesgo. La confianza metodológica de las revisiones fue predominantemente baja a moderada y existió heterogeneidad en poblaciones, definiciones de disbiosis y métodos microbiológicos. En conjunto, la evidencia respalda la disbiosis cervicovaginal como cofactor asociado al curso de la infección por VPH, aunque no permite establecer causalidad. Estos hallazgos justifican estudios longitudinales y ensayos que evalúen biomarcadores e intervenciones dirigidas al microbioma antes de su incorporación a estrategias clínicas o de tamizaje.
