NURSING CARE IN PREMATURE MEMBRANE RUPTURE AND OLIGOHYDRAMNIOS

Abstract

Maternal and perinatal morbidity, associated with premature rupture of membranes, constitutes an important social and health problem, so it is essential to estimate their frequency and magnitude. This article condenses nursing interventions in patient with a premature rupture of membranes by applying the nursing care process based on Dorothea Orem's theory, which is based on self-care, in which the level of dependence is defined to apply a medical intervention obstetric nursing. The methodology used is the case study, since it allows describing the pathology and management of a problem by intervening to achieve the health of a mother and her future child. A 19-year-old pregnant woman participated, with 29 weeks and 5 days of gestation, who through the assessment stage provided information on her current status and background. A diagnosis was made by means of a fern test where it reflects a premature rupture of the membrane that led to oligoamnios, manifesting a uterine fundus height of 19 cm and a threat of preterm birth. The oligoamnios puts at risk the adequate growth of the fetus, in addition it is not with a viable pregnancy to resort to a childbirth since it has less than 33 weeks of gestation. Dependent nursing care is proceed seeking absolute rest; and interde pendent maintaining a water balance, adequate food, treating constipation, infection prevention, and anxiety management that can affect sleep and comfort. The assiduity of nursing care achieves a positive response, by prolonging the gestation stage until a viable fetal maturation for life.
