Background: The diagnosis of inflammatory bowel disease in pediatrics requires a thorough understanding of its natural history, its various forms of presentation, as well as the different methods available for its study. Requesting the appropriate procedures avoids diagnostic delay and allows effective treatment of our patients.
Case Description: We present the case of a 5- year-old boy with no family or personal history of note, with diarrhea, diffuse abdominal pain, fever and anal fissure. After traditional endoscopic studies (gastroscopy and colonoscopy) and imaging studies such as small bowel transit and MRI of the abdomen and pelvis without being able to advance in the diagnosis, capsule endoscopy was requested.
Conclusion: Capsule endoscopy was important to establish the diagnosis and guide the treatment. He presented a very good response to adalimumab with total improvement of diarrhea and healing of the fissure.
