Tropical piomiositis by escherichia coli in patient with diabetes mellitus: atipic case, clinical and radiological aspects

Background: Pyomyositis is the purulent infection of the skeletal muscle that occurs by hematogenous spread, with formation of abscesses, the main etiological agent is E. aureus (90%) followed by beta hemolytic streptococcus group A and gram negative enteric bacilli (Ex. coli). It is common in patients immunocompromised by hematological malignancies, related or not with previous chemotherapy and AIDS. Clinical case: Male of 52 years hospitalized in the Honduran Institute of Social Security, Tegucigalpa, Honduras, main symptom of oppressive pain in the pelvis of 3 weeks of evolution, with irradiation to the anterior side of the right thigh, with inlammatory changes at this level, a week after pain and inlammatory changes in the left leg. Discussion: Compared with healthy people, patients with underlying medical conditions have higher rates of infection by gram-negative bacteria, multifocal disease and higher mortality. The state of immunosuppression is an important predisposing factor in the pathogenesis.
