Bilateral Thalamic Infarct by Commitment of the Artery of Percheron

Introduction: The Percheron artery originates from a single arterial thalamus-perforating trunk that provides bilateral supply to the paramedian thalami and the rostral mesencephalon. This is an anatomical variant that allows the bilateral irrigation of the thalami, from a common trunk of asymmetric origin in the posterior cerebral artery. Injuries in this area give rise to a variety of clinical manifestations. Case report: We present the case of a 89-year-old white female patient who was admitted to the Virgen de los Lirios Hospital in Alcoy, diagnosed with bilateral thalamic infarction of the Percheron artery. Discussion: Many factors for which this infarction is underdiagnosed among them is the underestimation of the frequency of this infarction together with the great clinical heterogeneity and also the low sensitivity of the computed tomography in the hyperacute phase. In general, the long-term prognosis is good, this varies with respect to the radiological patterns found, being more affected cases involving mesencephalon.
