Abstract

Introduction: The incidence of agenesis of the appendix is reported in one out of every 100,000 laparotomies performed for suspected appendicitis. Obviously the diagnosis is transoperative and usually creates a certain inconvenience in the medical staff because of the extremely rare condition. Clinical case: 17-year-old female with abdominal pain of 24 hours of evolution with suspected diagnosis of acute appendicitis versus inflammatory pelvic disease. Faced with this doubt, it was decided in common agreement with the parents to perform an exploratory laparotomy, finding a type III intestinal malrotation with subhepatic free cecum plus appendicular agenesis and both enlarged fallopian tubes with frank erythema of serosa. With these surgical findings we concluded the diagnosis of pelvic inflammatory disease. Conclusions: agenesis Appendiceal is very rare, many surgeons have never had the opportunity to see a case in his career, henee the importance of presenting this case stating that the diagnosis should not be performed until the ileocecal and retrocecal regions have been exhaustively explored.
