Giant uterine fibroids in nulliparous women: Report of two cases

Abstract

Background: Giant uterine fibroids are very rare benign neoplasms of uterine smooth muscle, thus defined by a greater weight of 11.3 kilograms and diameter greater than 20 centimeters may occupy the entire abdominal cavity. Case review: Two cases referred to the Maternity Hospital, Tegucigalpa, Honduras, from rural areas of the metropolitan region and south respectively are presented; the first: 43-year-old nulliparous forwarded by progressive increase in abdominal girth of three years of development also frequency and urgency of urination. Physical examination abdominal mass was palpated from symphysis pubis to 5 cm above umbilicus, irregular, hard, painless, still, the abdominal ultrasound showed solid mass about the size of 30 x 30 cm confirming the diagnosis of uterine giant fibroids, subtotal abdominal hysterectomy was performed. The second case: it was a patient of 74 year old with a history of diabetes mellitus type 2, referred because of abdominal mass progressive increase of 20 years of evolution, accompanied by pain and pelvic heaviness, physical examination mass was palpated in abdominal cavity of about 25 centimeters in diameter, round, hard, mobile, painful, abdominal ultrasound showed tumor of about the size of 26X24 centimeters, pelvic computed tomography reported giant fibroid and left ovarian cyst. subtotal hysterectomy, bilateral salpingo-oophorectomy was performed and antibiotic coverage for seven days. Conclusion: These cases of giant uterine fibroids longstanding can be avoided by strengthening the comprehensive care for women in places of first level, allowing early diagnosis and treatment.
