The present "in vitro" study was carried out at the Evangelical University of El Salvador, School of Dentistry, to determine the frequency of the 4 'conduit of the buccal mesio root of the first permanent upper molar, having examined 241 extracted pieces. Of the 241, six were excluded because they did not meet the essential requirements. The pieces studied were 235 of which 2.55% (6 pieces) presented calcification of the canals of the oral mesio root (MB). 11.06% (26 pieces) presented 2 apical forages with a duct, 23.40% (55 dental pieces) presented 1 apical foramen with two ducts, 28.08% (66 pieces) presented 2 apical forages and 2 ducts, 34.89% (82 pieces) presented an apical foramen and a duct. Therefore, it is concluded that the total percentage of the frequency of the 4th conduit of the buccal mesio root of the first permanent upper molar of extracted pieces was 51.48% (121 pieces).
