Educational intervention to strengthen a Just Culture at the Dr. Alejandro Dávila Bolaños Military Teaching Hospital, 2024

Biases in medical care can have serious consequences on patient safety and the implementation of a just culture; recognizing and addressing these biases can lead to improvements in the quality of medical care. The objective of this research is to evaluate the effectiveness of the educational intervention aimed at promoting the practice of just culture and patient safety culture among healthcare personnel of a Central American Polyclinic of the Hospital Militar Escuela Dr. Alejandro Dávila Bolaños, Managua, Nicaragua. This was a quasi-experimental study involving 140 participants who attended 8 in-person education sessions between August 2023 and April 2024. Data was collected through questionnaires administered before and after the educational intervention. The variables assessed were safety culture, punitive culture, just culture and biases. The results showed that the safety culture was the best understood topic due to the greater number of correct answers; on the other hand, the topics of biases and just culture reflected greater difficulty due to the lower number of correct answers. When applying for the McNemar test, 9 of the 25 questions yielded statistically significant results for improved knowledge. We can conclude that this educational intervention was partially effective; however, a knowledge gap was identified regarding the concepts of fair culture and bias.
