Choledochal cyst with obstructive symptomatology in a pregnant woman

Choledochal cysts are rare, in western countries an incidence of 1 per 100,000live births has been reported. In Asian countries, this incidence is greater than 1
per 100 live births. They are usually diagnosed during childhood and in 25% of patients
they present during their adult life. It is unusual for them to occur during pregnancy,
especially during the first trimester. A case report was made with an unusual presentation
of the non-causal clinical picture, of a pregnant woman in her first trimester with obstructive
symptoms, evolving to cholangitis. Ultrasonographic and blood chemistry studies indicated
an obstructive process. Cholangioresonance and endoscopic ultrasonography revealed a type
1C common bile duct cyst, a sphincterotomy was performed which showed drainage of bile.
After discharge from the hospital, the patient presented again cholangitis, an endoscopic
drainage was performed, an instrumental curettage for failed abortion and elective surgery
was performed at four months; resection of common bile duct cyst with biliodigestive
diversion by open surgery, without complications.
