Introduction. From the pain suffering of cancer patients, consult the WHO manual "Pain Relief in Cancer" that proposes 5 recommendations for analgesia. Modifications have been made to the initial approach for modifications to the management of acute pain, which goes in the opposite direction: from more severe to milder, but the fundamental principles remain. Objective. This study was planned with the primary objective of knowing adherence to the principles of the WHO analgesic scale in the management of postsurgical pain in elective major surgery at the Hospital Nacional Rosales in the period beginning in April 2018. Design and methods. An observational, descriptive, longitudinal design of agreement of the indication analgesia in patients of active major surgery at the Hospital Nacional Rosales since August 1, 2018 was specified. The indications of the center studied were compared against the ideal according to the proposed principles. Results. Of 360 patients, it was found that the most widely used active surgical surgery was hospital admission 75.3%. 215 (59.7%) patients received general anesthesia. The kappa concordance index for adherence to the analgesic principles for the management of post-surgical pain from the WHO at the Hospital Nacional Rosales was poor at the four moments measured, with an adherence rate of 0%, and no use of an analgesic scale. to assess pain. Conclusion. At the Rosales National Hospital, WHO's analgesic principles are not being followed in the management of post-surgical pain.
