Abstract

The project "Family and Community Health of San Martín" aims to create an integrative program of family and community health within the framework of primary health care, in urban and rural territories of the regions, where the Faculties of Health Sciences of the San Martín University Foundation are present, in order to contribute to community development. in addressing the social determinants of health. This is done through the development of curricular actions of the Extension or social projection in articulation with local communities, students, teachers and other actors present in the territories and linked to the Health System; allowing us to contribute to closing gaps by generating healthier lifestyles. This macro-project of national scope contributes to the fulfillment of multi-campus academic integration, to the strengthening of the institutional national identity and the cultural values that characterize the institutional presence in the national territory according to the Institutional Development Plan  Line 2, program 2.2. To this end, the project is structured in five phases: Phase 1  "Contextualization and design" of the program; Phase 2 "Diagnosis and planning"; Phase 3 "Implementation"; Phase 4 "Monitoring and evaluation" and Phase 5 "Visibility". Although the structuring of the first and second phases of the project is in charge of the Fundación Universitaria San Martín (FUSM), it is possible that in the later phases other faculties and national and international actors will be articulated or added to  the project, including higher education institutions, NGOs and embassies.
